This form can be printed out and sent by post or submitted by email.  If you require badges with more than one design, please fill in a form for each design.

Postage   POSTAGE If ordering by post, please add postage costs to your total. If ordering through PayPal, postage will be added at the checkout - orders outside UK will be subject to a surcharge. Orders up to £20 = £1.75 p&p. £20 - £50 = £2.50 p&p. £50 - £100 = £3.75. £100 and over = £5.00 p&p.

On receipt of your order & photograph, a proof of the badge design will be sent to you by email for final approval before the badges are made.  Badges will be made and despatched on receipt of payment.  To pay by PayPal, a payment button will be sent in an email (usually at the same time as the proof).
